Five questions worth asking

You have completed surgery successfully. Healing appears to be progressing well. The patient is eager to move forward.
Now comes the question:

Is the implant ready for the next step?

Most treatment decisions are straightforward. Others are less certain. As an implant clinician, you are often required to make important decisions based on the information available at that moment. The more objective information you have, the easier it becomes to move forward with confidence.

1. Can you trust your workflow to deliver consistent outcomes?

Successful implant treatment depends on many factors. Bone quality, patient health, anatomy and healing capacity can all influence the outcome. Some factors are beyond your control, others are not.

When treatment is successful, it can be tempting to attribute the result to a single factor. The implant design. The surgical technique. Primary stability. Or simply giving the implant enough healing time. In reality, outcomes are rarely determined by one factor alone.

This is particularly important to remember when evaluating risk. A low insertion torque does not automatically mean treatment will fail. Likewise, an implant placed with high insertion torque is not guaranteed to succeed. Given sufficient healing time, many implants will successfully osseointegrate. Similarly, patients presenting with one or more risk factors can still achieve excellent outcomes when the treatment plan is adapted accordingly.

The challenge is that every patient presents a unique combination of factors. Bone quality, healing capacity, systemic conditions, surgical complexity and treatment protocols can all influence the path to a successful outcome.

The goal is not to eliminate every risk factor. The goal is to recognise them and adapt your workflow accordingly. Experienced implant clinicians rarely rely on instinct alone. They follow a structured process that helps them identify risks, gather information and make treatment decisions based on objective findings whenever possible.

The question is not whether risk factors exist.

The question is whether your workflow helps you identify and manage them.

2. How do you know if healing is progressing as expected?

Once an implant has been placed, healing begins. The question is how closely you follow that process. Do you actively assess how the implant is progressing, or do you mainly rely on the healing time you originally planned?

In many cases, healing follows the expected timeline. But not always.

Two patients with similar treatment plans can heal at very different rates. Bone quality, systemic health, smoking and surgical complexity may all influence the healing process.

Time alone does not tell the whole story.

An implant that has healed for eight weeks is not necessarily more stable than one that has healed for six.

This raises an important question:

How do you know what is happening beneath the soft tissue?

Clinical examination and radiographs provide valuable information, but neither can directly show how implant stability changes throughout healing. The more objective information you have available during this phase, the easier it becomes to evaluate progress, identify potential concerns and determine whether treatment is moving in the right direction.

Time remains one of the most commonly used indicators of healing.

3. What information is guiding your treatment decisions?

Every implant clinician gathers information throughout treatment.

CBCT imaging helps you assess anatomy and plan implant placement. Insertion torque is commonly used to help assess primary stability at the time of surgery. Clinical experience plays an essential role in interpreting every case.

Every piece of information has value.

The challenge is that none of it provides the complete picture.

Once healing begins, some of the information available at surgery becomes less relevant to the decisions that follow. Healing may progress as expected, faster than anticipated, or require additional time.

At that stage, treatment decisions benefit from information that reflects the implant’s current status, not just the conditions present at placement.

Experience will always be central to implant dentistry. But experience is often strongest when combined with objective information.

The more you know about an implant’s current status, the more confidently you can decide whether to proceed, wait or adapt the treatment plan.

4. Can you explain your treatment decisions to your patient?

Imagine a patient asking:

“Will I have my new tooth before my son’s wedding?”

In moments like these, treatment decisions often carry emotional weight for both the patient and the clinician.

If healing is progressing as expected, treatment may be able to move forward confidently and in time for the wedding. If not, additional healing time may be the safer option. Communicating that decision is far more challenging. It requires not only sound clinical judgement, but also the ability to help the patient understand and trust your recommendation.

But how do you know whether treatment can move forward or whether more healing time is needed?

5. Is there a way to make these decisions more objective?

Successful implant treatment involves a series of decisions. Choosing a treatment plan. Following healing over time. Determining when to move forward. Explaining those decisions to patients.

The common thread running through all of these questions is information.
Traditionally, clinicians have relied on a combination of clinical experience, radiographs, healing time and insertion torque to guide treatment. All of these remain valuable components of the decision-making process. However, each provides only part of the picture.

Implant stability measurement adds another dimension. It provides repeatable, objective information about implant stability, both at placement and throughout healing. This information can help establish a baseline at surgery, support decisions regarding treatment protocols and monitor healing over time.

For some patients, the data may confirm that treatment can move forward as planned. For others, and even more importantly, it may indicate that additional healing time is the safer option

The value is not that it replaces clinical judgement. The value is that it supports clinical judgement with objective information.
Think back to the patient asking whether their final crown will be ready before their son’s wedding.

With objective information available, the conversation becomes easier. You can explain why treatment can move forward as planned, or why waiting a little longer may lead to a more predictable outcome. In either case, the recommendation is supported by more than instinct alone.

Because confidence in implant dentistry is not about having all the answers. It’s about having the information needed to make the right decision for each patient.
